About Home Modifications from TADQ

Many of our referrals come via My Aged Care portal and directly from Occupational Therapist. If you are a registered participant, call My Aged Care on 1800 200 422 or via their website and ask for TADQ to provide your services.

My Aged Care will coordinate a home mod assessment with Occupational Therapist and provide us with a referral number.

To access our home mod services, we need the Occupational Therapist completed Home Mod Assessment report, please send to : projects@tadq.org.au | tadassist@tadq.org.au
